What is Regenerative Medicine?

Regenerative Medicine is an exciting and innovative treatment option for orthopaedic conditions and sports injuries. These treatments utilize the bodys own naturally occurring healing mechanisms to help treat injuries and relieve pain. The two newest treatment options we offer for orthopaedic and sports injuries are Mesenchymal Stem Cell injections (Bone Marrow Aspirate Concentrate, BMAC) and Platelet Rich Plasma (PRP) injections. Collectively, these treatments are referred to as Regenerative Medicine.

Mesenchymal stem cells are the bodies own specialized cells that have the potential to develop into cells that can repair damaged tissue, ligaments or tendons. Essentially, these cells are the blank cells of the body that may adapt into the type of cells they are surrounded by. Each of us is born with these cells and we continue to produce them as we age.

Bone Marrow Aspirate Concentrate or BMAC stem cell treatment involves harvesting the bone marrow from the back of the pelvis and extracting the stem cells from that bone marrow. Once the BMAC stem cells have been isolated, they are then reinjected into the injured area.

When injected into an injured ligament, tendon or joint, these have the capability to take theform of the cells around them.

For instance, BMAC stem cells injected into damaged tissue have the capability to take the form of healthy tissue cells around them. This increases the likelihood of repair as many injuries lack the ability to heal well on their own.

The Regenerative Medicine Specialists at Olympia Orthopaedic Associates use only the bodies own naturally occurring cells and do not use donor cells or those from embryos or other sources.

Platelets are the component of blood and responsible for healing injuries. These platelets contain specialized proteins and growth factors that trigger the bodys natural healing response.

PRP treatment involves drawing blood from the patient and separating platelets and growth factors from the rest of the blood using a centrifuge. This process allows our specialists to extract the isolated platelets and inject a concentrated amount of PRP into an injured area.

Once injected, the platelets can expedite the healing process for certain injuries. This is due to specific proteins and growth factors contained in the platelet-rich plasma that decreases inflammation and can help damaged tissue or tendons.

BMAC stem cells and PRP are commonly used to treat inflammation, osteoarthritis, ligament and tendon injuries. Common injuries that our Regenerative Medicine Specialists treat include:

Research and clinical data have shown that PRP and BMAC stem cell treatments are extremely safe. Only your bodies own blood platelets or stem cells are injected into the injured area and the chance of the body rejecting its own blood or cells is extremely minimal.

When utilized in the right patients, Regenerative Medicine has shown to be highly effective.

In one study, published in The American Journal of Sports Medicine, 78% of those with osteoarthritis of the knee that received PRP injections showed a reduction in pain and increased function in the knee after one year.

The key to the effectiveness of these treatments is making sure that this treatment is right for you. Although Regenerative Medicine treatments have shown great results, there are some injuries that cannot be treated with BMAC stem cells or PRP such as complete tears of tendons or ligaments.

Background

Regenerative medicine is an emerging area of science that holds great promise for treating and even curing a variety of injuries and diseases. Regenerative medicine includes using stem cells and other technologiessuch as engineered biomaterials and gene editingto repair or replace damaged cells, tissues, or organs. Stem cell-based approaches are under development in labs around the world, and some have already moved into clinical trials. Such progress notwithstanding, much work remains to be done toward the development of safe and effective regenerative medicine products and to realize the full potential of this field.

The 21st Century Cures Act, passed in December 2016, established the Regenerative Medicine Innovation Project (RMIP) to accelerate the field by supporting clinical research on adult stem cells while promoting the highest standards for carrying out scientific research and protecting patient safety. In recognition of the integral role of the Food and Drug Administration (FDA) in the successful development of this field, NIH is leading the RMIP in coordination with FDA.

The 21st Century Cures Act authorizes $30 million in federal awards over four years (20172020) for the RMIP. Importantly, the Act requires award recipients to match the federal funds provided with at least an equal amount of non-federal funds.This matching requirement will amplify the federal investment and could help stimulate collaboration and develop key partnerships across the public and private sectors.

Congress allocated $2 million for the RMIP in FY 2017. To make optimal use of these funds, researchers were invited to submit applications for competitive revisions to support clinical studies involving adult stem cells. Eight awards were issued in September 2017.

On December 6-7, 2017, the NIH and FDA hosted a public Regenerative Medicine Innovation Workshop that brought together key stakeholders to explore the state of regenerative medicine clinical research involving adult stem cells with a focus on approaches to the development of safe and effective products. Video recordings from each session are available. The deliberations at this workshop were instrumental in helping to identify some of the major needs, opportunities, and challenges in the regenerative medicine field.

Input from the workshop and other sources informed the development of the next round of funding opportunity announcements (FOAs) published in August 2018. These FOAs solicit new investigator-initiated projects, which may either involve a clinical trial or entail late stage Investigational New Drug (IND)- or Investigational Device Exemption (IDE)-enabling clinical research. What Is Regenerative Medicine?

Regenerative medicine is the catchall term for a burgeoning field of medicine that focuses on harnessing the bodys innate ability to heal itself in the management and treatment oforthopaedic injuries. Two of the most common procedures that fall under the umbrella of regenerative medicine is stem cell therapy and platelet-rich plasma (PRP) injections. While many clinicians believe that these procedures hold great promise, they are still in the very early stages of study and regulatory approval.

The ultimate goal of regenerative medicine in orthopaedics is to use the innate healing ability of a patients own body to regenerate diseased or damaged tissues. While regenerative medicine as a whole has applications well beyond the muscles, bones and connective tissues of the musculoskeletal system, orthopaedic regenerative medicine is specifically focused on delivering a specific set of cells to the site of an injury. These cells, through their natural anti-inflammatory and healing properties, in turn, act on the damaged tissue to help drive recovery.

Platelet-rich plasma (PRP) is the end product of a patients own blood that has been extracted and refined to create a far higher concentration of platelets than would normally be present. Best known for facilitating the clotting of blood, platelets, which contain hundreds of proteins called growth factors that reduce pain and inflammation, are now being recognized for the crucial role they play in the healing process. This concentrated preparation is injected into the site of injury and the platelets are allowed to go to work promoting healing in the injured area. PRP injections are always created from a patient's own blood, so there are never any foreign substances being introduced to the body during the procedure; it is simply a patients own blood that has been processed, without additives, to contain high levels of platelets, our bodys naturally occurring healing cells.

Stem cell therapy is a developing orthopaedic treatment modality that aims to harness the power of the bodys own stem cells to promote healing and recovery. Adult stem cells, which are found throughout the body, have many regenerative and anti-inflammatory properties and possess the unique ability to differentiate or change themselves. This allows stem cells to grow into a wide variety of cell types, including muscle, nerve, cartilage and even bone. Many orthopaedic surgeons believe that stem cells may have the ability to treat common soft tissue injuries and conditions, such as torn cartilage, arthritis, and muscle tears.

There is evidence to suggest that stem cell therapy has the potential to repair and strengthen damaged tissues throughout the entire musculoskeletal system, but the science is not yet conclusive on the effectiveness of the treatment. Each treatment is unique to the patient and results can vary from case to case. Furthermore, there are not yet clearly defined clinical protocols for how and when to deploy stem cell treatments in the management of orthopaedic conditions. To that end, there are research studies and trials currently underway around the world to better understand if and how these procedures can impact common orthopaedic injuries and conditions. As part of his clinical practice, Dr. Chris OGrady offers regenerative medicine procedures like stem cell therapy alongside traditional treatment modalities to ensure his patients have the best chance of success.

Anyone suffering from a musculoskeletal injury or disorder could be a candidate for regenerative medicine treatments. There is strengthening evidence to suggest that regenerative medicine could help promote healing and recovery, regardless of whether a patients pain is the result of chronic wear-and-tear or acute injury. Dr. OGrady commonly uses regenerative medicine to treat the following conditions:

-Arthritis: Arthritis literally translates to inflammation of the joint, and describes any disease process which leads to cartilage loss. Arthritis in the knee is very painful because the injury to the cartilage of the knee can eventually lead to bone-on-bone contact. This leads to increased stiffness, pain, swelling and decreased the range of motion of the knee.

-Meniscal Injuries: The meniscus is a pliable, rubbery disc that distributes body weight equally across the knee joint. This connective tissue can tear if the joint undergoes stress or sudden impact, preventing normal functioning of the knee and causing pain and inflammation.

-Ligament Injuries: The knee has four major ligaments: the anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), medial cruciate ligament (MCL), and lateral cruciate ligament (LCL). Sprains or tears to these ligaments can occur gradually over time or through acute injury to the joint; in any case, these injuries cause pain and instability in the knee and may need to be treated surgically. However, regenerative medicine can be used alongside or in place of traditional surgical treatments in some cases.

-Rotator Cuff Tears: The rotator cuff, a group of four muscles that attach the humerus (upper arm bone) to the shoulder blade, is one of the most crucial components of the shoulder. It allows the muscles and tendons of the shoulder to lift and rotate the humerus and is essential to almost every basic and complex movement we require of the joint. The rotator cuff can be injured through acute injury or it can tear gradually over time; in any case, injury to the rotator cuff is very painful and severely limits the shoulders ability to move and function as intended.

-Arthritis of the Shoulder Joint: Arthritis describes any disease process which leads to cartilage loss. Cartilage serves an incredibly important role in joints of the body, as it cushions space and prevents bone-on-bone contact. Arthritis can be very painful because the wearing down of cartilage causes bones to grind against one another. This leads to increased stiffness, pain, swelling and decreased use of the shoulder.

-AC Joint Separation: The acromioclavicular (AC) joint is the point in the shoulder where the collarbone (clavicle) and the acromion of the shoulder blade are connected by ligaments. An AC joint separation occurs when these ligaments are strained or separated, typically from a sharp blow to the shoulder. Grade 1 separations generally involve a sprain of the AC ligament, while Grade 6 separations involve total tears of the ligaments and noticeable deformity in the shoulder.

-Labral Tears or Degeneration: The labrum is a ring of firm fibrous tissue that surrounds the shoulder socket (glenoid) and stabilizes the joint. When the labrum is torn or degenerates, it can cause pain, stiffness, a decrease in athletic ability, and a popping or clicking sensation with movement.

While the safety and efficacy of regenerative medicine procedures are still under review, it is believed that there are some major advantages to these procedures versus traditional treatments, particularly surgery. First, these procedures are far less invasive than traditional surgical approaches and are therefore done in the outpatient setting. There are no incisions required for these procedures and, as such, the risks of infection and complication are dramatically lower than surgery, and recovery is very minimal. Oftentimes the only immediate side-effect of the procedure is soreness at the site of injection. Second, stem cell therapy requires very little, if any, pain medication during recovery, so there is no risk of adverse side effects to opioid medication. Given that we are currently in the midst of an opioid epidemic on par with some of the worst drug scourges in our nations history, keeping patients off of narcotics during recovery is no small matter. Finally, stem cell therapy relies on the bodys own cells to drive recovery, so there are no foreign chemicals, additives or implants being injected into your body.

Nanotechnology and Nanoscience offer immense benefits to humans with an effective amalgamation of nanotechnology & stem cells.

Gene Therapy is used to treat inherited Muscular disorder, cardiovascular disorder, HIV, cancer etc. In stem cell transplants, stem cells replace cells damaged by chemotherapy or disease or as a way for the donor's immune system to provoke immunity against some types of cancer and blood-related diseases, such as leukemia. Cellular Therapy is internationally recognized for its novel approaches in treating blood-related disorders like leukemia, lymphoma, myeloma, and other life-threatening diseases. The stem cell transplantation of hematopoietic stem cells (HSCT) in which the allogeneic hematopoietic stem cells are harvested from healthy donors of same species and autologous stem cell from the patient itself. Both therapies use high dosage cytotoxic medication in order to induce higher remission rates against malignant diseases. Autologous HSCT preferably used in relapsed malignant high-grade lymphoma and Allogeneic HSCT preferred for therapeutic effect against acute leukemia with unfavorable prognosis in a high percentage of patients. The Recent developments based on the expansion of the donor pool for allogeneic stem cells in order to reduce dosage as well as the chemotherapeutic toxicity of allogeneic transplantation with sustainable anti-leukemia efficacy.

Classical methods of gene therapy include transfection. It became inefficient and limited mainly due to the delivery of the gene into actively proliferating cell s in-vitro. Gene therapy utilizes the delivery of DNA into cells by means of vectors such as biological in-vitro or viral vectors and non-viral methods. Several kinds of viruses vectors used in gene therapy are the retrovirus, adeno-associated virus, and herpes simplex virus. While other recombinant viral vector systems have been developed, retroviral vectors remain the most popular vector system for gene therapy protocols and widest application due to their historical significance as the first vectors developed for efficient gene therapy application and the infancy of the field of gene therapy.

Spinal discs are soft and compressible that helps cushion the spinal column, which allows the spine to flex, twist, and bend. Although it can occur anywhere along the spine, it most often occurs in the lower back and neck. The pain can occur throughout the spine but in some instances affects the intervertebral disc.

Surgery is not always successful and there is a slight risk of damage to the spine or nerves, and risk of infection. New techniques from regenerative medicine using a patients own stem cells has shown good results with regeneration and rebuilding of the network of cells that make up the injured disc.

This is a common form of chronic foot pain that occurs between the ball of the foot and the heel. There is a thick connective tissue on the bottom of the foot, called the plantar fascia, which connects the ball of the foot to the heel. This plantar fascia supports the arch of the foot and can become strained from a number of different sources, including overuse, tight calf muscles, and poor foot placement. The damage forms tiny tears along the ligament, which is the likely source of pain. Treatments usually target the underlying condition, and then the symptoms of pain.

BMAC is a concentrate of regenerative stem cells obtained from the patients own bone marrow. The physician removes a small amount of the patients bone marrow and uses it to generate a powerful concentrate that is injected into the treatment area.

Unlike other cells of the body, bone marrow cells are undifferentiated, which means they have the ability to transform themselves into a variety of tissue types. When injury occurs, the quantity of regenerative cells needed for tissue regeneration may be inadequate. With BMAC, the injection of regenerative cells provides a more robust healing of damaged tissue and aids in growth and repair by accelerating the bodys natural healing mechanism. While the full benefits of BMAC are still unknown, BMAC has been shown to reduce swelling, relieve pain, and enhance healing of articular cartilage and bone.

Numerous conditions can be considered for treatment with BMAC. Currently, moderate to severe osteoarthritis and severe tendon injuries show promising results.

In general, PRP may be more appropriate for mild to moderate osteoarthritis or tendon injuries. BMAC may be reserved for more challenging cases such as moderate to severe osteoarthritis or when more potent effects are desired.

With the patient sedated, the bone marrow aspiration site is locally numbed so minimal pain is felt. Bone marrow is removed from the back of the pelvis (hip) at the posterior iliac crest. The concentration of bone marrow (stem cells and healing components), also known as the bone marrow aspirate concentrate (BMAC), is injected into the treatment area. Patients go home the same day.

Unlike cortisone/steroid shots which simply mask symptoms (and may damage tissue with repeated injections), BMAC targets the root of the problem and attempts to heal the tissue.

Most patients require only a single BMAC treatment. However, if a patient experiences significant relief that plateaus, they may consider a second BMAC injection months later.

For the first 2-3 days, swelling and discomfort may occur in the injected area. By the end of the first week, these symptoms usually begin to resolve. Physical therapy is started within a few days of the treatment, to optimize BMAC effects and facilitate recovery. Patients respond to BMAC treatment along varying timelines.

Bone marrow derived cancer (such as lymphoma) and active systemic infection are contra-indications. Other types of cancer may be a contra-indication and approval must be obtained from the patients oncologist.

No. While there is evidence showing the positive effects of BMAC treatment on tendon, soft tissue, and cartilage injuries, BMAC is not covered by insurance companies at this time.

Most patients notice some level of improvement by 2-6 weeks following their BMAC treatment. Increases in stability and strength are typically reported, along with the decreases in pain. A second level of benefit may be obtained between 6 weeks and 3 months following BMAC. Patients should remain active with a physical therapy program and strengthen surrounding muscles during this period.

Platelet-Rich Plasma (PRP) is produced from your own blood. Platelets are cells in our body that contain growth factors. These growth factors stimulate the normal wound healing process, such as when your skin heals after a scrape. For PRP treatments, we concentrate your platelets (to over 5x more platelets than your normal blood) and deliver them to the area of interest, to help heal injured tissue. Some PRP treatments are done under ultrasound guidance to confirm placement of the PRP in the correct tissue.

PRP is indicated for injuries that have failed to heal despite traditional treatments. PRP treatments can be performed in any musculoskeletal structure, including muscles, tendons, joints and ligaments. Some examples include: partial tendon tears, muscle strains, ligament sprains/partial tears, articular cartilage injury, and chronic tendon injuries.

To prepare PRP, blood is taken from your arm with a special kit. This is similar to a normal blood draw. The blood is then placed in a centrifuge that separates the platelets from the blood, creating the PRP. The entire PRP treatment process takes about 45 minutes.

Many patients achieve successful outcomes with only one treatment, especially for soft-tissue problems. In some cases, a series of three treatments is required to achieve significant results. This is particularly true for joint treatments. Each treatment is spaced several weeks apart. There is no limit to the number of treatments you can have; however, if youre not seeing significant improvement after three treatments, you should consider other forms of treatment.

Since your own blood is used, there is no risk for transmitted blood-infections. PRP has a strong antibacterial effect so the risk of local infection is minimal. It is possible to have increased soreness or pain for several days after the treatment. You may be prescribed pain medication to help with this.

PRP treatment is a fairly new procedure, and most insurance companies have not incorporated it yet to their list of approved procedures and do not consider it a reimbursable expense. PRP treatment fees include the PRP kit, blood draw, use of centrifugation machine, disposable equipment, ultrasound guidance (if needed), and the actual procedure. A splint for support may be indicated in some cases (additional fee, likely with some insurance coverage).

PRP stimulates healing of the injured tissue by activating your bodys natural healing potential. On average, patients report more than 50% improvement at 6 weeks and up to 100% improvement at 12 weeks after treatment. PRP treatment may eliminate the need for other treatments such as long-term medication or surgery.
