Stem cell therapycan bea very controversial issue, but now some veterinarians are using new techniques to harvest those cells.

The cutting edge procedure helps fight degenerative diseases and has only been performed a few times in Missouri.

Experts say regenerative medicine using stem cells is a less invasive and more cost effective alternative for dogs suffering from osteoarthritis and cartilage injuries.

Googus is an 8 year old Boxer mix diagnosed with degenerative myelopathy.

This terminal disease affects the spinal cord causing loss of control in the hind legs.

"Even though they're unable to use their back legs they're still normal in their brain and they just don't understand why they can't walk," said Dr. Stephen Williams, Animal Health Center. "There's just not a good connection and transmission from the nerves to the back legs."

But new technology could slow, if not stop, its progression. Dr. Williams is using stem cell therapy to counteract this and other degenerative diseases in dogs.

"The stem cells from the patient are the ones that are going to benefit that same patient versus trying to take stem cells from a different dog and putting them in this dog," said Dr. Williams. "By harvesting the stem cells from the fat versus people have heard of stem cells from umbilical cords and stuff like that we're taking it from the fat tissue and harvesting those and actually activating with a fluorescent light."

Once the fat is extracted it's a two hour process to prepare the new stem cells. Those are then injected back into the patient along with platelets that work with the immune system to fight the disorder.

San Francisco, CA, October 22, 2013 A study performed at Children's Hospital Los Angeles found no evidence that stem cell therapy improves vision for children with optic nerve hypoplasia (ONH). Their results are reported in the Journal of the American Association for Pediatric Ophthalmology and Strabismus (AAPOS).

ONH, an underdevelopment of optic nerves that occurs during fetal development, may appear either as an isolated abnormality or as part of a group of disorders characterized by brain anomalies, developmental delay, and endocrine abnormalities. ONH is a leading cause of blindness in children in North America and Europe and is the only cause of childhood blindness that shows increasing prevalence. No treatments have been shown to improve vision in these children.

With no viable treatment options available to improve vision, ophthalmologists are becoming aware that families with children affected by ONH are travelling to China seeking stem cell therapy, despite lack of approval in the United States and Europe or evidence from controlled trials. The American Association for Pediatric Ophthalmology and Strabismus has also expressed its concern about these procedures. In response to this situation, pediatric neuro-ophthalmologist Mark Borchert, MD, Director of both the Eye Birth Defects and Eye Technology Institutes in The Vision Center at Children's Hospital Los Angeles, realized that a controlled trial of sufficient size was needed to evaluate whether stem cell therapy is effective at improving optic nerve function in children with ONH. He agreed to conduct an independent study when asked by Beike Biotech, a company based in Shenzhen, China, that offers treatment for ONH using donor umbilical cord stem cells injected into the cerebral spinal fluid.

Beike Biotech agreed to identify 10 children with bilateral ONH (ages 7-17 years) who had volunteered to travel to China for stem cell therapy and who agreed to participate in the study; Children's Hospital was to find case matched controls from their clinic. However, only two case-controlled pairs were evaluated because Beike Biotech was only able to recruit two patients. Treatments consisted of six infusions over a 16-day period of umbilical cord-derived mesenchymal stem cells and daily infusions of growth factors. Visual acuity, optic nerve size, and sensitivity to light were to be evaluated one month before stem cell therapy and three and nine months after treatment.

No therapeutic effect was found in the two case-control pairs that were enrolled. "The results of this study show that children greater than 7 years of age with ONH may have spontaneous improvement in vision from one examination to the next. This improvement occurs equally in children regardless of whether or not they received treatment. Other aspects of the eye examination included pupil responses to light and optic nerve size; these did not change following treatment. The results of this research do not support the use of stem cells in the treatment of ONH at this time," says lead author Cassandra Fink, MPH, program administrator at The Vision Center, Children's Hospital Los Angeles.

Confounding the trial was that subjects received additional alternative therapies (acupuncture, functional electrical stimulation, and exercise) while receiving stem cell treatments, which was contrary to the trial protocol. The investigators could not determine the effect of these additional therapies.

"This study underscores the importance of scientifically testing these procedures to validate them and also to ensure their safety. Parents of afflicted children should be aware that the science behind the use of stem cell technology is unclear. This study takes a step toward testing this technology and finds no beneficial effect," says William V. Good, MD, Senior Associate Editor, Journal of AAPOS and Clinical Professor of Ophthalmology and Senior Scientist at the Smith-Kettlewell Eye Research Institute.

--Findings in elderly mice offer insight into helping elderly people recover from burns

Newswise Johns Hopkins researchers, working with elderly mice, have determined that combining gene therapy with an extra boost of the same stem cells the body already uses to repair itself leads to faster healing of burns and greater blood flow to the site of the wound.

Their findings offer insight into why older people with burns fail to heal as well as younger patients, and how to potentially harness the power of the bodys own bone marrow stem cells to reverse this age-related discrepancy.

As we get older, it is harder for our wounds to heal, says John W. Harmon, M.D., a professor of surgery at the Johns Hopkins University School of Medicine, who will present his findings to the American College of Surgeons Surgical Biology Club on Sunday at 9 a.m. Our research suggests there may be a way to remedy that.

To heal burns or other wounds, stem cells from the bone marrow rush into action, homing to the wound where they can become blood vessels, skin and other reparative tissue. The migration and homing of the stem cells is organized by a protein called Hypoxia-Inducible Factor-1 (HIF-1). In older people, Harmon says, fewer of these stem cells are released from the bone marrow and there is a deficiency of HIF-1. The protein was first discovered about 15 years ago at Johns Hopkins by Gregg L. Semenza, M.D., Ph.D., one of Harmons collaborators.

Harmon and his colleagues first attempted to boost the healing process in mice with burn wounds by increasing levels of HIF-1 using gene therapy, a process that included injecting the rodents with a better working copy of the gene that codes for the protein. That had worked to improve healing of wounds in diabetic animals, but the burn wound is particularly difficult to heal, and that approach was insufficient. So they supplemented the gene therapy by removing bone marrow from a young mouse and growing out the needed stem cells in the lab. When they had enough, they injected those supercharged cells back into the mice.

After 17 days, there were significantly more mice with completely healed burns in the group treated with the combination therapy than in the other groups, Harmon says. The animals that got the combination therapy also showed better blood flow and more blood vessels supplying the wounds.

MANILA, PhilippinesStem cell therapy should only apply to skin grafting for burn patients and not for anti-aging purposes, according to the Food and Drug Administration (FDA).

Up to now, there is no evidence that stem cell therapy has anti-aging effects, said FDA acting Director General Kenneth Hartigan-Go.

The FDA recognizes only hematopoietic (pertaining to the formation and development of blood cells) stem cell transplantation, corneal resurfacing with limbal stem cells and skin regeneration with epidermal stem cells as generally accepted standards of health care.

Asked if this meant anti-aging stem cell therapies would not be allowed in the Philippines, Go said: It means that if the health claim is for burn patients, requirements that need to be submittedlike clinical trial reportsare expected to be complete, whereas if the claim is for anti-aging, then the requirements may be more extensive and intensive considering that products for anti-aging claims are still controversial.

Facility accreditation

The FDA said it had been continuously collaborating with the Department of Healths Bureau of Health Facilities and Services (BHFS) in accrediting facilities that deal with human cells, tissues, and cellular and tissue-based products (HCT/Ps), laboratory and therapeutic activities or services.

As of Sept. 24, 51 facilities have applied for accreditation, and 11 of these have been inspected by the BHFS, the FDA said.

No automatic approval

Submission of an application, however, does not necessarily result in automatic approval.

The FDA guidelines require that all stem cell and cellular-based treatments offered in the country should first pass the agencys standards for safety, efficacy and quality.

Stem Cell Therapy for Parkinson’s Proves Safe in Primates

In a step that brings stem cells closer to the clinic, researchers in Japan have found that transplanting reprogrammed stem cells into the brains of primates elicits little rejection by their immune systems.

Induced pluripotent stem cells (iPSCs) are created when skin cells, for example, are genetically reprogrammed to an embryonic-like state. This kind of stem cells holds great potential for the treatment of disease, since the cells are genetically identical to the patient they are taken from.

However, studies in rodents have suggested that the immune system may still recognize cells derived from iPSCs as foreign, and mount an attack on them. This has cast doubt on the feasibility of similar cell therapy for humans.

To test this in an animal more closely related to humans, researchers studied macaques. Using cells taken from the monkeys mouths or from their bloodstream, the researchers created iPSCs which they then, in turn, transformed into neurons. These neurons were of a specific kind: dopamine-producing neurons, the type depleted by Parkinsons disease.

Each monkey got six injections of these neurons into its brainsome which had been made from their own cells and others which were from another individual and therefore mismatched. The team could then see what kind of immune response each type produced.

Over subsequent months of observation, the monkeys showed very little immune response to transplants of their own cells. Their immune response was much higher in response to cells from another monkey.

The team also tracked how well the neurons survived after transplantation. They found that even when there was an immune response from the primate, the dopamine-producing neurons survived. The study is published today in Stem Cell Reports.

Trials using iPSCs to treat people with Parkinsons disease could therefore be on the horizon. These findings give a rationale to start autologous transplantationat least of neural cellsin clinical situations, says senior author Jun Takahashi of Kyoto University.
